Note: forms may be revised prior to the next scheduled examination and you may be required to re-submit your application on new forms. Prior to any examination notarized applications, and required work history must be completed and returned to the Charleston MHS&T office for review by the Mine Inspector's Examining Board. Once the Board review the applications for eligibility, you will be notified of the exam date, time and location.

Office of Miners’ Health, Safety and Training Several study guides, and the West Virginia Mining Law Book are available for purchase for a fee, and can be requested at the address listed above, ATTN: Publications. Or you can order these publications on line. Our agency now accepts publication orders and payments (visa and master card only) on line! If you wish to place an on line order for any of our publications, do so through our publication order site. Publication charges are based on the item ordered, plus a $2.00 postage and handling fee per volume. See the list below for a listing of available materials. For prompt shipment, please enclose a cashiers check or money order with your request. Please state clearly which publications you wish to order.
