EFFECTIVE JUNE 3, 2013, THE  WV OFFICE OF MINERS' HEALTH, SAFETY AND TRAINING WILL ISSUE VIOLATIONS FOR NON-COMPLIANCE OF THE REQUIREMENTS IN TITLE 56, SERIES 17 (COAL DUST/ROCK DUST RATIO).

EFFECTIVE APRIL 17, 2013, THE WV OFFICE OF MINERS' HEALTH, SAFETY AND TRAINING WILL ISSUE VIOLATIONS FOR NON-COMPLIANCE OF THE REQUIREMENTS IN 22A-2-63(e) (EXTENSION OF CERTIFICATE OF APPROVAL).

THE WV OFFICE OF MINERS' HEALTH, SAFETY AND TRAINING WILL BEGIN ISSUING THE SAFETY SENSITIVE PERSONNEL CARD ON TUESDAY,  JANUARY 22, 2013. 

YOU MUST VISIT ONE OF OUR OFFICES TO HAVE YOUR PHOTO MADE AND CARD ISSUED. A COMPLETED SAFETY SENSITIVE PERSONNEL FORM AND A $10 FEE IS REQUIRED.
